Registered dietitian Lisa Dawes, MS, RDN, CDCES will provide you with a superbill following your initial consult with her. This document will include diagnosis codes, MNT (medical nutrition therapy) provider number and NPI number, which you may submit to your insurance company for reimbursement. Many insurance companies now recognize the benefits nutrition counseling and are willing to pay for them, and require these provider numbers. Ms. Dawes does not bill insurance through her office so that she may continue to offer her services at affordable rates.

Yes (for these situations only):
Medicare and United Healthcare with a diagnosis of diabetes. Prior to scheduling your appointment with Ms. Dawes, please call your insurance carrier to see how many hours of Medical Nutrition Therapy (MNT) you have available to you. If you have used your MNT hours for the year, you will be billed directly by Ms. Dawes and payment is owed at the time of service.
Physicians Choice HMO (authorization required)
Dignity Health EPO (authorization required)
For all other insurances:

Payment for services rendered are due at the time of the visit.
